Chronic Kidney Disease Help
WCRx can help with the treatment of Chronic Kidney Disease
WCRx is a Pharmacy & Clinic in Tallahassee, Florida that has been serving the community for many years. We have served thousands of patients in Leon County and counties throughout Florida, as well as multiple states. We know there’s nothing more important than you or your family’s well-being, which is why we strive to provide personalized care to help you and your family with your healthcare decisions.
Through our step wise 360-care model, we vet and screen our patients for point of care, treatment, review and follow up. This prevents patients need for urgent or emergency crisis intervention and tax dollar back breaking ER visits. We have been working for over a decade with ambulatory care practice models with Physicians, Pharmacists, ARNPs, RNs and Researchers to ensure access to optimal and cost-effective options for healthcare. WCRx applies measurable methods with technology to execute nationwide preventative care methods for communities in both metropolitan and rural areas. Our clinicians have access to current laboratory values along with other pertinent information and proprietary methods to create a unique experience for the patient, focusing on a preventative directive approach with state-of-the-art technology to ensure a 360-care model is always digitally implemented. As a participating member in our preventative care program, patients will have guaranteed healthy outcomes.
For those CKD who are stage 3-4 or even 5 are usually eligible for CCM (Chronic Care Management) which is a monthly management program. WCRx can provide a CGM if diabetic, PGx, and remote patient monitoring. Additionally, we provide opportunity to see if they are eligible for "Rayaldee".
Rayaldee is used to treat people with serious kidney disease whose bodies make too much of a certain natural substance (parathyroid hormone-PTH). High amounts of PTH can affect other minerals in the body (such as calcium, phosphorus) that are needed for strong bones.
Also, if eligible when we start a "Chronic Care Management Program for them,. it allows us at no cost to them to see what opportunities are available for transplant and or alternative treatment to reverse kidney failures.

What is Remote Patient Monitoring (RPM)?
Remote patient monitoring is a technology to enable monitoring of patients outside of conventional clinical settings, such as in the home or in a remote area, which may increase access to care and decrease healthcare delivery costs.
WCRx can help you monitor and manage your health from the comfort of your home.
The RPM (Remote Patient Monitoring) program wirelessly sends the information from your compatible medical device directly to your health file for remote review and monitoring by our physicians with real time health data to help make decisions on how to provide the best care for you. Regular monitoring of chronic conditions helps with keeping your measurements within range, which can prevent, or delay emergency room visits and hospitalizations.

To challenge out-dated methods of treatment and diagnosis for Chronic Diabeteic Illnesses by getting to the root of the problem instead of just pumping people with insulin and medications. We found that many of these conditions could easily have been prevented had there been a proper monitoring system with nutritional regimens in place all along!
