For those CKD who are stage 3-4 or even 5 are usually eligible for CCM (Chronic Care Management) which is a monthly management program. WCRx can provide a CGM if diabetic, PGx, and remote patient monitoring. Additionally, we provide opportunity to see if they are eligible for "Rayaldee".
Rayaldee is used to treat people with serious kidney disease whose bodies make too much of a certain natural substance (parathyroid hormone-PTH). High amounts of PTH can affect other minerals in the body (such as calcium, phosphorus) that are needed for strong bones.
Also, if eligible when we start a "Chronic Care Management Program for them,. it allows us at no cost to them to see what opportunities are available for transplant and or alternative treatment to reverse kidney failures.

Remote Patient Monitoring for Nephrology
What is Nephrology?
Nephrology is a specialty of medicine and pediatric medicine that concerns with study of the kidneys, specifically normal kidney function and kidney disease, the preservation of kidney health, and the treatment of kidney disease, from diet and medication to renal replacement therapy (dialysis and kidney transplantation). Nephrology also studies systemic conditions that affect the kidneys, such as diabetes and autoimmune disease; and systemic diseases that occur as a result of kidney disease, such as renal osteodystrophy and hypertension.
WCRx's Remote Patient Management is targeted to cardiology patients who have been identified as candidates for Chronic Care Management (CCM) services. Each patient will also receive a kit that includes a wireless transmission enabled weight scale, blood pressure cuff and pulse oximeter.
